Looking for PHP coder

hey,
i'm in need of a PHP coder who is able to add a little upload script to my site(www.tomhuveners.net)

I need an upload thing, on url ie. tomhuveners.net/upload.php orso...
with small login or not..
but it has to do the following:
- Input Field for Name of Project
- Input Field for image location with (browse button)
- and another field under it:
for image URL, only one of the 2 needs to be filled in, max size of image should be editable in php orso..
- Add button so it ads it to the mainpage.

Who can do this?
