Need a little help

I've been working with php for about a month, and getting better. I have this form and from my knowledge it seems right, the form submits to the processing php page, and the php does not return any parse errors, however I never receive the email from the form, I omitted my email and zipped both the .htm form and the .php processing page HERE HERE. If anyone can tell me what I'm doing wrong, because I know its something quite trivial.
