Please give me your price and your previous work on the following:

A script I will be giving away needs some styling. The script is a donation script that shows a bar of how close you are to your donation target. Preferably this should match www.donatometer.com. It needs to be sidebar-integratable. By this, I mean it won't be a full blown template, so nothing too wide. It just needs to be able to fit into a small side-content section. Something around 180px in width.

It needs a heading bit at the top which will say something like "Donate to Us". Underneath this will be the area for the donation bar. The design needs to include what the bar looks like full and what the bar looks like empty. Go to www.washyourhandsplease.com to see the concept of the bar. There also needs to be a last 5 donations section (with a heading style for the column titles). The last 5 donations will be grouped using the following headings:

Name, Amount Donated, Date Donated

Then there needs to be a 'See All Donations' and 'Make A Donation' buttons, and finally, a 'Powered by Donatometer' (with 'Donatometer' being linked) at the bottom, like a footer.

It must use web-friendly fonts.
