free contact form with file attachment?

Hello,
I'm looking for a free contact form with file attachment.

I've been searching all day on google and hotscripts, and I can't find anything decent.
I'm looking for something really simple, no javascript validations or whatever, just a working contact form with like 1 field (or more) and attachment, I can do the rest of the fields myself, but not the attachment part.

I found a few but they don't work or have to many futures wich I can't filter out.

I hope someone can link me to one?
