Need PHP coder for form with Paypal

I'm looking for someone who can code a form using PHP and integrate paypal with it.. the info would just need to be sent to an email.

I'd do it myself, but I'm working on other projects and I don't have time. I'm paying very well for this to be done as soon as possible. Doesn't have to have anything fancy.

Basically, all that needs to be done is take all their information, and submit it to an email. However, you need to see how much to charge them via Paypal from the drop downs, and charge them accordingly.
