  Old  Health Survey

Hey all,

A new semester has begun at school, and with it, new classes. This semester I get to learn all about health

Right now we are doing a project in there and I need to have about 15 people complete this survey. Preferably they would be around 15 years old, but I invite anyone to answer. Any help appreciated


Here are the questions. Answers should be 'yes', 'no', or 'sometimes':

Yes, 1 cheat day each week though

Yes, no drugs tho, that's just nasty.

Yes, bbing is a huge hobby

Sometimes

Yes, more than 3 :P

It's more like 6-9, depending on what day of the week it is.

I get anywhere from 6-10 hours of sleep a night. Depends on what I have going on and what I had going on the night before.

I do not eat as healthy as I should but heh... I eat what I want to.

Yes, before school every morning.
Do you use alcohol or drugs?
I do drink on occasions but I do not use drugs.
Do you mantain your recommended weight?
I have no idea what mine is... I am 6'3 180lbs. Take it from there...
Do you smoke?
No Sir.
Do you have 20-30 minutes of vigourous activity 3 times a week?
Yes, I play basketball for my high school and practices can go anywhere from an 1 and a half or 2 hours and 30 minutes. We pracitce everyday we don't play...

No. I sleep maybe 5-6 hours at night then usually take a small nap during the day.

No. Unless a periodic Slimfast drink counts

Alcohol - Yes, Drugs - hell no
