Contact form

I'm thinking about implementing a project submission form into my web site TapwaterProductions.com. It would be pretty involved seeing as there is a lot of information I need to obtain from a potential customer to get their project started right away. Because I'm a music producer and not a professional web programer, I wouldn't know how to create something this complex. I was wondering how much a service like this would cost. It might need to be updated and revised regularly as it gets tested.
