I have PHP for dummies on my shelf and I do not recommend it, I would personally look into buying a couple of books

PHP:-
O'Reilly - Programming PHP 2nd ed (Good for begginers guides)
O'Reilly - PHP Cookbook 2nd ed (Handy for quick fixes or programming solutions)
PHP & MySQL for dummies, (not great but meh, its on the shelf)

MySQL:-
O'Reilly - MySQL Cookbook 2nd ed (Once again handy for problems, no good for general learning thou)
